The Arizona Attorney General settled an investigation into SimonMed's upfront-billing and delayed-refund practices.
The Attorney General’s office alleged that SimonMed charged patients upfront based on “good-faith estimates” but delayed refunding overpayments — in some cases for more than a year — and did not proactively offer a pay-after-adjudication (“tokenization”) option.
Resolved by consent judgment (announced January 2026): $50,000 in civil penalties, $20,000 in restitution, and $20,000 in costs and fees. SimonMed must issue refunds within an average of 60 days after claim adjudication and actively offer the tokenization option. The consumer-restitution complaint deadline was set for April 30, 2026.
