How to file a complaint

Where a complaint goes depends on the issue:

  • Billing / consumer practices — your state Attorney General's consumer-protection division. In Arizona, the AG's office took action on SimonMed's billing; consumers can file a complaint at azag.gov.
  • Facility conduct / quality — your state health department. In Arizona, the Department of Health Services handles facility complaints (azdhs.gov).
  • Insurance / claims problems — your health plan's grievance process first, then your state Department of Insurance.
  • A data-breach notice — keep the letter; it explains the credit-monitoring you're entitled to and your rights.
You can choose where you get scanned

A referral names a place, but you are generally free to have imaging done at a different in-network facility. Ask the referring office to send the order to the provider you choose, or ask your insurer for in-network imaging centers near you. If a scan is time-sensitive and one provider can't schedule it, another often can.
