Diazepam overdose during MRI — Thompson Peak Imaging Center, Scottsdale

The lawsuit alleges that during a 2021 MRI at SimonMed’s Thompson Peak Imaging Center in Scottsdale, a patient was given 50 mg of diazepam (Valium) — which the plaintiff’s counsel described as roughly five times the FDA’s maximum recommended dose — instead of MRI contrast media. In court documents SimonMed admits the patient was provided diazepam instead of contrast media, but denies the other allegations. The remaining allegations are unproven.

The incident was reported in an October 2023 ABC15 investigation, which also found that SimonMed had operated an imaging facility unlicensed and unregulated by the Arizona Department of Health Services under a private-office licensing exemption, and referenced a separate May 2022 MRI safety incident at SimonMed’s Avondale location.
